How this works

The 56-day response deadline

Report put online after its 56 days

Organisations that receive a report must reply to the coroner within 56 days, saying what they will do or why they will do nothing. We show whether a response is published on judiciary.uk; a reply may have been sent and not yet published.

judiciary.uk put this report online on 13 Oct 2026, after the 56 days for replies had ended (24 Aug 2026). These dates can’t tell us whether a reply was due before it went online. A reply may have been sent and not yet published.

The coroner’s concerns

are as set out in Box 6 above. Shiree had a shunt to manage her longstanding hydrocephalus and presented to Lincoln County Hospital on 30 th August 2025, with headache and increased absence seizures. Neurosurgical advice was sought on 1.9.25 but not followed. No urgent eye assessment to look for papilloedema (swelling of the optic discs caused by raised intracranial pressure) was requested as advised. There was no recording of headache frequency or severity, and very limited recording of seizures to help guide urgency of management. These issues all led to a delay in recognising that Shiree had persistent raised intracranial pressure, due to a shunt blockage, such that there was a delay in transfer to Queens Medical Centre, which did not occur until 13.9.25. 11 COPIES AND PUBLICATION OF THIS REPORT I have a duty to send a copy of my report to every Interested Person who in my opinion should receive it.
